Before you attend your appointment with CMMS

you'll need to

a) download & read the Information Sheet pdf - this contains information on Preparations, Payments, Cancellations and Results for your medical.

b) download & complete the Address Form pdf - you must bring this fully completed when you attend your appointment (one form per family).

c) read the HIV Testing Information sheet pdf.

d) read the FAQ (Frequently Asked Questions) pdf.

e) download a copy of the 'How To Find Us' map if you need one.

f) If you have made an appointment by telephone

please confirm via the contact form names, dates of birth of all attendees + a contact address & telephone number.
